Today’s health news is dominated by the stark findings of the Lucy Letby inquiry, which concludes that repeated failures in hospital leadership and safeguarding left babies exposed to avoidable harm. Alongside that reckoning, separate cases focus attention on child safety in care settings and urgent responses when very young children are at risk.

A judge-led inquiry finds a “complete failure” by management at the Countess of Chester Hospital to protect babies during the period when neonatal nurse Lucy Letby murders and attacks infants. The report says warnings are mishandled at multiple levels, and that preventable shortcomings in oversight and response leave vulnerable newborns at risk.

The Thirlwall inquiry finds that some baby deaths may have been avoidable if police had been contacted sooner. It says earlier intervention could have prevented some attacks, sharpening criticism of the hospital’s response to mounting concerns as Letby goes on to be convicted of murdering seven babies and attempting to murder seven others.

A two-year-old girl, Sadie Salt, dies after a suspected choking incident while eating lunch at a nursery in Radlett, Hertfordshire. She is taken to hospital and dies two days later, and prosecutors launch a case over the circumstances surrounding the incident at the Mini Learners nursery.
